Having gone through thousands of films, I constantly find film report cards inside the cans -- especially educational's and those that came from school libraries.
They were meant be helpful for the next projectionist so that they would take care of the problem and to keep the film in top shape.

Most "free loan" libraries in the UK required their users to return a comment sheet with the film.

Some 16mm educational library prints at the school I went to had "Claw Marks" written on the can label - so I assume there was no report card system there.

I just made up some small check lists that I put in with all my prints then I made a spread sheet with any comments on it for quick reference.
