It sounds as if he was on the floor reaching for something that had fallen under the chair and the power operated leg rest came down on him, trapping and crushing him.

Any further investigation to the root cause of this accident - namely the chair's manufacturer?
There should be some sort of safety measure built-in. The link didn't mentioned that whether that safety system had been bypassed, or it never existed ever since?

BTW probably not a pleasant way to die, thinking about Final Destination movie.😨

It seems that all the seats have now been removed from the chain and "And an inquest, which ended in a verdict of accidental death, was told the reclining chair had been missing a bar that would have allowed him to be released by hand." Details in link in the original post by Maurice.

Apparently the guy was searching for his cell phone which had fallen under the seat.
